****Please complete all necessary forms with the assistance of your healthcare provider and return them to your child’s school at the start of each school year, so that the school nurse and school staff have everything they need to help your child while at school.
***Severe BEE STING or FOOD allergies***
If it is known that your child has any allergies (food or bee sting) that may require the use of Benadryl or an EpiPen, it is very important that you share this information with your child’s school nurse. Please fill out the questionnaire and return it at your earliest convenience. There is also the new Order for Administering Prescription Medication in School form. According to Wisconsin law and the Board of Education Policy, medication cannot be administered during school hours unless a signed physician and parent order is on file at the school. A new physician and parent order must be signed annually. Your child's physician may also fax a signed medication order if you choose to not fill out the medication order form.

***Seizures***
Students who have a seizure disorder must have a current Emergency Care Plan on file. Please complete the Seizure Disorder Questionnaire and return it to your child’s school at the start of each school year. The information on the questionnaire will be used by the school nurse to complete or update the Emergency Care Plan.
There is also a new Order for Administering Prescription Medication in School form. If your child requires emergency medication at school this must be completed. According to Wisconsin law and the Board of Education Policy, medication cannot be administered during school hours unless a signed physician and parent order is on file at the school. A new physician and parent order must be signed annually. Your child's physician may also fax over a signed order if you choose to not fill out the medication form.

***Diabetes***
If your child has diabetes and would need help with management in school please have their medical provider fill out the Diabetes Medical Management Plan form developed by the Wisconsin Department of Public Instruction. An updated plan is required at the start of each school year.
The Order for Administering Prescription Medication in School form is available if medications are to be given in school. According to Wisconsin law and the Board of Education Policy, medication cannot be administered during school hours unless a signed physician and parent order (for insulin and Glucagon) is on file at the school. A new physician and parent order must be signed annually. Your child's physician may include the medication order on the Diabetes Medical Management Plan.

***Asthma***
If your child has asthma, they must bring in an asthma action plan completed and signed by their physician and parent/guardian along with a signed Orders for Administering Prescription Medication form. According to Wisconsin law and the Board of Education Policy, medication cannot be administered during school hours unless a signed physician and parent order is on file at the school. A new physician and parent order must be signed annually.
The asthma action plan and/or medication form allows your child to use their inhaler at school or self-carry (if allowed at school and agreed by the parent/guardian and physician).
***Over-the-counter medications***
The school shall administer over-the-counter medication for a maximum of three consecutive days. It is the District's philosophy that if the student requires over-the-counter medication for more than three days, they should be seen by a physician.
Each school building has a limited stock of over-the-counter medications for the school year, therefore, parents are highly encouraged to bring in their over-the-counter medications, and they can be kept in the nurse's office for your child to use. Nonprescription medication (e.g., Tylenol) may be administered to students by school personnel with written instructions and consent by the parent/guardian or adult student. The non-prescription must be delivered to school personnel by the student's parent/guardian or adult student.
For your student to receive any school stock over-the-counter medications at school (Tylenol, ibuprofen, Tums, cough drops), a parent/guardian must have the medical consent filled out online on Skyward. If that consent is NOT filled out, your child will NOT be able to take any school stock medications.
