Please complete the form to nominate an LHS graduate for inclusion in the LHS Athletic Hall of Fame.

(Once individual has been nominated by filling out the online form, the nominee will be contacted and asked to fill out a bio packet which will then be scored by the screening committee. Individuals only need to be nominated 1 time to be sent a bio packet. Multiple nominations do not increase likelihood of induction.)
