Offices hours are 8:00 a.m. - 4:30 p.m., Monday through Thursday; 8:00 a.m. - 4:00 p.m. Friday
Please bring a birth certificate so that we can verify your child's date of birth, as well as medical information so forms on medical history can be completed.
