Registration

  Registration Information

Parents with school-age children who are new to the community, and those parents moving within the school district are encouraged to register at their children’s new school as soon as possible. Parents with questions about school boundaries may call the Thomas A. Lenk Educational Services Center, 422-6013, for assistance. If a student has special education needs, additional information is available by contacting Pupil Services at 422-6015.

K-12 school registration is accomplished any time during the school year by making an appointment with the school principal or secretary. Summer and four-year-old kindergarten registration is obtained by calling the Thomas A. Lenk Educational Services Center, 422-6000.

The two junior high schools and the high school conduct registrations approximately two weeks before school begins in the fall. Elementary school secretaries and principals are in the buildings at least two weeks prior to the start of school in September, and one week after school ends in June.

A certified birth certificate, baptismal certificate, passport, or immigration certificate, as well as medical record is required by law.  A recent report card from the last school attended is helpful. A child must be five years old on or before September 1, for entrance into kindergarten, or six years old on or before September 1 for entrance into first grade. Early admission procedures begin with a request to the Pupil Services Department (422-6015) where paperwork will then be forwarded to you. The deadline for early admission is May 1 prior to the beginning of the school year.

Since good physical health is vital for a successful school career, it is strongly recommended that children have a physical examination prior to entering kindergarten, third, sixth, and ninth grades. Wisconsin Statute requires that all students in grades K-12 be immunized against:

Diptheria                                                            Pertussis
Tetanus                                                             Polio
Varicella (chicken pox)                                     Mumps
Measles                                                             Rubella
Hepatitis B (as of the 2003-04 school year)

At registration time, parents will be given a form, which will request information on the dates (day, month, and year) when the immunizations were administered.
